Membership Terms and Conditions

• Your first membership payment is due on the day you enroll. Recurring payments will automatically be charged on the same date each month thereafter.
• Memberships require an initial 3-month commitment. After the initial commitment, the membership will continue month-to-month until canceled. A 30-day notice is required for cancellation prior to the applicable renewal period.
• Unused monthly services may be rolled over. The dollar value of an unused service may also be applied toward eligible services not included in your membership, with the member responsible for any difference in price.
• Membership credits will not be forfeited while your membership remains active and in good standing. However, memberships are designed around consistent monthly care. Accumulating more than three unused monthly services within a calendar year may result in review or cancellation of the membership.
• Membership services, accumulated credits and member privileges may be redeemed only while the membership is active and payments are current. Membership benefits cannot be used while an account is frozen, suspended or delinquent.
• If a membership payment is declined, member benefits and redemption of accumulated credits may be temporarily suspended until the account is brought current. More than two declined or missed monthly payments within a calendar year may result in cancellation of the membership.
• Treatment selection is subject to provider approval and may be modified when necessary based on skin type, medical history, medications, contraindications, recent treatments, sun exposure or other safety considerations.
• Some treatments require specific spacing between appointments. Membership does not override the recommended treatment interval or clinical guidelines for any service.
• Member discounts apply to eligible regularly priced services and products and cannot be combined with other promotions, specials, packages or discounts unless otherwise specified.
• Memberships, monthly services and accumulated credits are for the enrolled member only and are non-transferable.
• Membership payments and accumulated membership credits have no cash value and cannot be exchanged for cash or refunded.
• Standard Laser Beauty Works cancellation and no-show policies apply to all membership appointments.
• Memberships are offered on a limited basis to ensure appointment availability for our members. Once membership capacity has been reached, new enrollment may be placed on a waitlist until space becomes available.

Laser Beauty Works reserves the right to modify membership offerings, eligible treatments or benefits when necessary. Active members will be notified of any material changes to their membership.
